Tracy Milner, OT Reg. (Ont.)

President / Occupational Therapist

Tracy is a Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario and a member of the Ontario Society of Occupational Therapists (OSOT). She has worked as an Occupational Therapist in the private sector for over fifteen years. She has completed both her Master of Clinical Science degree in Occupational Therapy and Honours Bachelor of Arts degree in Kinesiology at The University of Western Ontario.

She is experienced in occupational therapy assessment and treatment as well as case management of clients with severe or catastrophic injuries, such as traumatic brain injury, multiple trauma, complex orthopedic, psychological impairments, and spinal cord injury. She works with both adults and children. She is a PGAP certified clinician. She is experienced in the preparation of Life Care Plans.

She is a member of the OSOT Auto Sector Committee and Attendant Care Task Force and sits on the Board for the Ontario Rehab Alliance. Her leadership experience has included Supervisor of OT Services at a large disability management company and Managing Director of Dynamic Rehabilitation. She acquired the Ontario business of Dynamic Rehabilitation in June 2008 and founded Complex Injury Rehab Inc. with Heather Condello.

Heather Condello, OT Reg. (Ont.)

VP of Operations / Occupational Therapist

Heather is a registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario and a member of the Ontario Society of Occupational Therapists. She has completed a Master of Clinical Science degree in Occupational Therapy and a Bachelor’s degree in Kinesiology.

Heather has over fifteen years’ experience as an Occupational Therapist having spent the majority of her career as a community based therapist working within the auto insurance sector; however, also spending time practicing in outpatient burns, amputations and complex orthopedics and working with neurologically impaired clients including the prescription of assistive technology for such clients.

She is currently the Vice President of Operations and a practicing Occupational Therapist with Complex Injury Rehab, living in Durham Region and practicing throughout Durham region, the Greater Toronto Area, and Peterborough counties. Her current practice includes occupational therapy in home assessment (including assessment of attendant care needs, home safety assessments, and comprehensive physical, cognitive and psychosocial assessments), discharge planning assessments, case management and occupational therapy treatment for clients with a brain injury, spinal cord injury, burns, amputations, chronic pain and orthopedic injuries. Heather has a special interest in paediatrics. She also completes life care plan reports and is a Canadian Certified Life Care Planner.

Lisa Iliadis, OT Reg. (Ont.)

Clinical Services Manager / Occupational Therapist

Lisa is a Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario. She has over fifteen years of experience and has provided expert testimony. Lisa is a Professional Associate with McMaster University`s School of Rehabilitation Science. In her practice, Lisa has worked with client’s recovering from acquired brain injury, orthopaedic injuries, burns, amputations and other complex multi-system injuries. She has performed in-home assessments to identify functional limitations and care needs arising from physical, cognitive, perceptual and psychological impairments. She has provided occupational therapy treatment in hospital, home, school, work, retirement homes and in the community at large. Lisa is a certified provider of the Progressive Goal Attainment Program (PGAP) and a graduate of psychology. She is positioned to address adjustment difficulties, depression, anxiety and other emotional factors that impact recovery. Lisa practices in both English and French.

Sylvia Haycock, OT Reg. (Ont.)

Clinical Supervisor - East / Occupational Therapist

Sylvia is a Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario. She graduated from Queens University with an Honours Bachelor of Science in Occupational Therapy. In addition, she holds an Honours Bachelor in Human Kinetics from the University of Windsor. She is an adjunct lecturer at University of Toronto Occupational Science and Occupational Therapy Program.

Sylvia has over thirteen years’ experience working as an Occupational Therapist with extensive experience working with clients with neurological conditions. She is experienced in functional evaluations of individuals who are experiencing difficulties performing their essential tasks of daily living due to illness and/or injury. She has over nine years of experience working in the auto insurance sector completing comprehensive occupational therapy assessments focusing on functional, cognitive, psychosocial and emotional aspects of injury. She is experienced in brain injury assessment and rehabilitation both in the community and a hospital setting. Sylvia is experienced in complex seating and is an ADP Authorizer for Seating and Mobility Products. She is NDT trained, has Advanced Bobath Certification and provides Hand and Upper Extremity Therapy and Splinting. She is trained in modalities including functional electrical stimulation and offers MyndMove Therapy. Areas of practice include head injury, spinal cord injury, complex pain, orthopaedics, and working with clients with psychosocial impairments. Sylvia is currently Occupational Therapist Supervisor East in addition to practicing as an Occupational Therapist in homes and in a clinic setting with Complex Injury Rehab Inc.. She provides services to clients in the Greater Toronto Area.

Shannon Orr, OT Reg. (Ont.)

Clinical Supervisor - Barrie and North / Occupational Therapist

Shannon is a qualified Occupational Therapist licensed to practice in Ontario and is registered with the College of Occupational Therapists of Ontario (COTO). She graduated from University of Toronto with a Master’s of Science in Occupational Therapy. In addition, she holds a Bachelor of Science in Therapeutic Recreation from Dalhousie University. She is a member of the Ontario Society of Occupational Therapists. Shannon has over ten years’ experience working as an Occupational Therapist. She practices in Barrie and surrounding areas.

Shannon is experienced in comprehensive functional evaluations of individuals who are experiencing difficulties performing their essential tasks of daily living due to illness and/or injury.

Her current practice includes occupational therapy in home assessment (including assessment of attendant care needs, home safety assessments, and comprehensive physical, cognitive and psychosocial assessments), discharge planning, and occupational therapy treatment for client with brain injury, spinal cord injury, chronic pain, orthopaedic injuries and various psychological diagnoses. Shannon also has additional training in brain injury, hand therapy, microcurrent point stimulation, comprehensive functional cognitive assessment, and functional capacity evaluations. She is an ADP Authorizer for Seating and Mobility.

Colleen Worsley

Manager of Community Engagement

Colleen has recently completed a social work degree with a background in psychology, cognitive science and nutrition. She has more than 20 years' experience in the rehabilitation healthcare field providing direct care and management support in both clinical and community settings for individuals with traumatic brain injury. Colleen has experience with the process involved to develop teams of multi-disciplinary professionals to support client needs. She combines her interest in the neurological system with her studies in wellness and healing to identify client strengths and needs and to develop individualized and comprehensive rehabilitation programs with her clients. Colleen sits as a board member for the Brain Injury Society of Toronto where she is proud to be part of an organization that fosters community connections and integration for survivors of brain injury.

Johanna Baker, OT Reg. (Ont.)

Manager of Health & Wellness Programs / Occupational Therapist

Johanna Baker is a duly qualified Occupational Therapist in good standing with the College of Occupational Therapists of Ontario (COTO) and a member of the Ontario Society of Occupational Therapists (OSOT). She obtained her Bachelor of Arts degree (Criminology with minor focus in Psychology) from Simon Fraser University in 2000 and in 2006 obtained her Master of Occupational Therapy degree from the University of British Columbia. In 2015, Ms. Baker obtained qualifications as a Canadian Certified Life Care Planner through the International Commission on Health Care Certification (ICHCC). Ms. Baker has over ten years' experience providing rehabilitation consultation services with adults in various settings, including hospitals, schools, and the community. She is skilled in the rehabilitation of adults with various health issues, including orthopaedic and musculoskeletal impairments, mood disorders, post-traumatic stress, chronic pain, dementia, neurological conditions, and cardiovascular conditions. Ms. Baker is also committed to continual learning and professional development with a special interest in vocational health, safety, and wellness; she has been employed as Manager of Health & Wellness Programs and Consulting Occupational Therapist at Complex Injury Rehab since July 2017.

Michelle Jacob, OT Reg. (Ont.)

Occupational Therapist

Michelle holds a Master of Science in Occupational Therapy from Queen’s University. She is a Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario. Michelle practices in English, Hindi, and Malayalam. Michelle practices in the east end of Toronto and Durham Region.

In her practice, Michelle has worked in hospital and community settings. She has provided occupational therapy assessment and treatment within the Ontario auto insurance sector to clients with acquired brain injury, orthopaedic, musculoskeletal injuries and pain conditions. She has worked with children and adults alike to enhance functioning through the provision of compensatory strategies, cognitive remediation, and education. She is also versed in hand therapy including static and dynamic splinting.

Michelle has NDT training and is a certified provider of the Progressive Goal Attainment Program (PGAP). She is positioned to provide functional restoration programming to address pain, depression, and post-traumatic stress disorders. Michelle is particularly passionate about Mindfulness Therapy and has continued to pursue learning in the area of mindfulness and yoga therapy in order to incorporate this into her work with clients.

Chantal Doerig, OT Reg. (Ont.)

Occupational Therapist

Chantal Doerig is a Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario. She graduated from Dalhousie University in 2004 with a Bachelor of Science degree in Occupational Therapy, and from Dalhousie University in 2001 with a Bachelor of Science degree in Kinesiology. Chantal practices in the west end of Toronto, Mississauga, Oakville, Brampton, Milton and surrounding areas.

Chantal has over thirteen years’ experience working as an Occupational Therapist with extensive experience providing home and community-based occupational therapy in the areas of traumatic brain injury, orthopaedic and musculoskeletal injuries and psychosocial impairments. In her practice, Chantal has worked in the auto insurance sector since 2006, with experience completing comprehensive occupational therapy assessments focusing on functional, cognitive, psychosocial and emotional aspects of injury. She has provided occupational therapy services in the field of mental health, geriatric medicine and neurological conditions in hospital and rehabilitation based settings. Chantal has also provided workplace education and training in the areas of ergonomics, return to work planning and job coaching. She is fluent in English and French.

Sarah Light, OT Reg. (Ont)

Occupational Therapist

Sarah Light is a Registered Occupational Therapist in good standing with the College of Occupational Therapists and a member of the Canadian Association of Occupational Therapists.

She has obtained a Bachelor of Health Science degree from Western University and a Master of Science in Occupational Therapy from McMaster University. Sarah has extensive experience with adults and older adults following acute stroke, brain injury and musculoskeletal impairments in inpatient and community settings. Sarah also has international experience, most recently working as a Senior Stroke Occupational Therapist in London, England.

Sarah is experienced in comprehensive neurological assessment and treatment (including cognitive, perceptual, upper extremity and psycho-emotional aspects to assessment), functional assessment/intervention in the home and community, and comprehensive psycho-social assessment. Sarah is currently practicing as an Occupational Therapist with Complex Injury Rehab Inc. and provides services to clients in Toronto and surrounding areas in English, Polish and German. Her current practice includes occupational therapy in home assessment (including assessment of attendant care needs, home safety assessments, and comprehensive physical, cognitive and psychosocial assessments), discharge planning assessments and occupational therapy treatment for clients with brain injuries and other neurological conditions, chronic pain and orthopedic injuries.

Christie Kerr, OT Reg. (Ont.)

Occupational Therapist

Christie is a Registered Occupational Therapist in good standing with the College of Occupational Therapists and a member of the Canadian Association of Occupational Therapists. She has completed her Masters of Science degree in Occupational Therapy at the University of Western Ontario and Honours Bachelor of Arts degree in Psychology at the University of Western Ontario.

Christie has extensive experience in transitioning clients with complex disabilities, such as spinal cord injuries and neurological conditions, from hospital to community-based destinations. She also has experience completing in-home assessments, and providing client-centered treatment to maximize client safety and independence in the home and community. She is experienced in seating and mobility prescription, and is an ADP Authorizer. Christie is currently practicing as an Occupational Therapist with Complex Injury Rehab Inc. and provides services to clients in the Greater Toronto Area and surrounding areas.

Lorie Ann Tirone, OT Reg. (Ont)

Occupational Therapist

Lorie Ann is a duly qualified Occupational Therapist in good standing with the College of Occupational Therapists of Ontario (COTO) and a member of the Ontario Society of Occupational Therapists (OSOT). She obtained her Honors Bachelor of Science degree (Biology and Psychology) from the University of Toronto in 2000 and later graduated in 2004 from Queen’s University with a Bachelor of Science degree in Occupational Therapy. Lorie Ann has over 13 years of experience working with adults in various settings including hospitals and in community. She is skilled at completing comprehensive assessments to identify functional limitations resulting from physical, cognitive and psychosocial impairments. She has provided treatment to individuals recovering from various medical conditions including orthopaedic injuries, neurological conditions, burns and amputations. Lorie Ann is committed to continuing education with a special interest in acquired brain injury and cognitive rehabilitation and has completed the Level 1 & 2 certification training in Neurorehabilitation at Brock University.

Ayushi Dhingra, OT Reg. (Ont.)

Occupational Therapist

Ayushi is a Registered Occupational Therapist in good standing with the College of Occupational Therapists, and a member of the Ontario Society of Occupational Therapists. She completed her Masters of Science in Occupational Therapy at McMaster University. Ayushi practices within the Greater Toronto Area and Durham Region. Her current practice includes occupational therapy in-home assessment (including assessment of attendant care needs, home safety assessments, and comprehensive physical, cognitive and psychosocial assessments), and occupational therapy treatment for clients with a brain injury, chronic pain, orthopaedic injuries, and mental health conditions. Ayushi is a BrainFx 360 Administrator and has completed various courses including two Level 1 ABI training courses through OBIA, NDT Upper Extremity training, Assisted Suicide Intervention Skills Training (ASIST), Mental Health First Aid, and Gentle Persuasive Approaches in Dementia Care. Richard Chui, OT Reg. (Ont.)

Occupational Therapist

Richard is a Registered Occupational Therapist in good standing with the College of Occupational Therapists and a member of the Ontario Society of Occupational Therapists. Richard is currently a practicing Occupational Therapist with Complex Injury Rehab in the Greater Toronto Area. He practices in both English and Cantonese.

Richard has over five years’ experience in comprehensive assessment that incorporates physical, functional cognitive and psycho-social assessment. He has additional training in Goal Management Training for Executive functions, PGAP, Microcurrent Point Stimulation (pain management modality), Orthotics, Motivational Interviewing, and BrainFx 360 assessment.

Richard's current practice includes occupational therapy in home assessment (including assessment of attendant care needs, home safety assessments, and comprehensive physical, cognitive and psychosocial assessments) and discharge planning assessments. He utilizes concepts from his training in the PGAP program and experience with psycho-education and graded exposure therapy in working with clients with psychosocial impairments. Richard administers the BrainFx assessment with clients with mild to moderate traumatic brain injury and develops recommendations for treatment using results from this comprehensive assessment. He is experienced in the prescription of assistive equipment including working with a variety of populations post-discharge to manage equipment and assistance needs. He also has experience in ergonomic assessment and with return to work programs for adults with a variety of diagnoses.

Jenna Hunt, OT Reg. (Ont.)

Occupational Therapist

Jenna is a qualified Occupational Therapist licensed to practice in Ontario and registered with the College of Occupational Therapists of Ontario (COTO). She holds a Master’s of Science in Occupational Therapy from University of Alberta as well as a Bachelor of Science in Immunology and Infection. She has been working as an occupational therapist since 2010 and working in the auto sector since 2016. Jenna practices in the Barrie area.

Jenna has experience working in long term care and community settings. She is experienced with safety assessments and home modifications as well as provision of mobility devices. She has performed a variety of cognitive assessments and implemented treatment plans to maximize functional performance and safety within the home. She has worked as a case manager coordinating treatments and services to ensure best possible outcomes for clients. She is currently practicing part time performing in-home assessments, return to work planning, and occupational therapy treatments for clients with acquired brain injuries, orthopaedic injuries, and chronic pain.

Maha Wakim, OT Reg (Ont)

Occupational Therapist

Maha is a registered Occupational Therapist within Ontario. She is a member of the College of Occupational Therapists of Ontario and of the Ontario Society of Occupational Therapists. She completed her undergraduate Bachelors of Science in Psychology and Linguistics at the University of Toronto and holds a Master of Science in Occupational Therapy from Queen’s University. Since graduating Maha has continued to engage in continuing education including conference attendance, completion of the Level 1 OBIA Neurorehabilitation Training Program and attendance at a seating clinic to increase knowledge of seating and mobility. She practices in English, Arabic, and French.

Maha practices as an Occupational Therapist in the auto-insurance industry within the Barrie area. Through this position, she has gained experience with a range of clients including children, teenagers, and adults with varying physical, psycho-social, and cognitive symptoms. Thus she is well versed with the cognitive, psychosocial, and functional aspects of recovery from traumatic injuries. She also has extensive experience working with clients from the pediatric population on motor, cognitive, functional, and psycho-social development. Maha believes that empathy and kindness should be the driving forces in any interaction. Her ultimate professional goal is to remain client-centered, evidence-based, and focused on function, independence promotion, and meaningful occupational engagement.

Helen Leimonis, OT Reg. (Ont.)

Occupational Therapist

Helen is a Registered Occupational Therapist in good standing with the College of Occupational Therapists and a member of the Canadian Association of Occupational Therapists. Helen has over ten years of experience working as an Occupational Therapist including extensive experience with orthopaedic and musculoskeletal impairments, chronic pain, and acquired brain injury in community settings. She has experience working with both adults and children.

She is experienced in conducting comprehensive In-Home Assessments, Assessment of Attendant Care Needs, Functional Abilities Evaluations, Ergonomic/ Work-Site Assessments, and Cognitive and Physical Demands Analyses. Helen has undergone extensive training in the field of Acquired Brain Injury through the Ontario Brain Injury Association and Brock University. Additionally, she is certified in CogMed, Progressive Goal Attainment, AMPS, Irlen Screening, and in providing Cognitive Behaviour Therapy. Helen has completed the AMA Guides to Impairment Rating, 4th Edition and has completed the Canadian Certified Life Care Planner Pre-Certification Program through the University of Florida.

Helen is currently practicing as an Occupational Therapist with Complex Injury Rehab Inc. and provides services to clients in York and Durham Regions and north Toronto. Her current practice includes occupational therapy in-home assessment (including assessment of attendant care needs, home safety assessments, and physical, cognitive and psychosocial assessments), discharge planning assessments, and occupational therapy for clients with a brain injury, musculoskeletal and/or orthopaedic injury, and chronic pain.

Bani Ahuja, OT Reg. (Ont.)

Occupational Therapist

Bani is a qualified Occupational Therapist, registered with the Canadian Association of Occupational Therapists and the College of Occupational Therapists of Ontario. She graduated from McMaster University in 2010 with a Master of Science in Occupational Therapy and from the University of Toronto in 2008 with an Honours Bachelor’s Degree in Health Studies and Psychology. She has experience with assessment and treatment of individuals with physical, cognitive, and psychiatric disorders.

She has worked with children and adults alike to enhance functioning through the provision of compensatory strategies, cognitive remediation, and education. She is also versed in medical-legal assessments, brain injury, and paediatrics (including Autism Spectrum Disorder, Sensory Processing Disorder and other learning disorders).

She regularly attends courses, seminars and conferences to update her skills and expertise in assessment, rehabilitation and therapeutic intervention. Bani is trained in the Progressive Goal Attainment Program (PGAP) intervention, AMA Guides to Impairment Rating, 4th Edition, Handwriting Without Tears, and is a certified by BrainFx Administrator (CBA). She is a Canadian Certified Life Care Planner. She holds faculty appointment at the School of Occupational Therapy and Occupational Science at the University of Toronto. Bani practices in English, French, Hindi, Punjabi, Urdu, Gujarati and Arabic.

Michelle Stinson, OT Reg. (Ont.)

Occupational Therapist

Michelle is a registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario and a member of the Ontario Society of Occupational Therapists. She completed her post-secondary education at the University of Toronto which includes a Bachelor of Science degree in Biology and Psychology, a Bachelor of Science degree in Occupational Therapy and a Masters of Education degree.

Michelle has 18 years of experience as an Occupational Therapist and has worked in both the hospital, community and auto sectors. She also has extensive experience in seating and mobility prescription, home and vehicle modifications, in home assessments and implementing and monitoring quality management programs. She has served as an appointed Professional Member for the College of Occupational Therapists of Ontario Discipline Committee since November 2013.

She is currently practicing part-time as an Occupational Therapist with Complex Injury Rehab providing services within the Greater Toronto Area. Her current practice includes occupational therapy in home assessment (and occupational therapy treatment. Michelle is also an ADP Authorizer for seating and mobility aids. She is currently completing studies to become a certified Life Care Planner.

Kylie Averell

Occupational Therapy Assistant

Kylie is an Occupational Therapy Assistant providing support to individuals who have suffered complex / catastrophic injuries. She graduated from Georgian College Occupational Therapist Assistant/Physiotherapist Assistant Program in 2014, and her experience ranges from acute to chronic care in the hospital and long term care settings. Kylie has been working as an Occupational Therapy Assistant for 3 years. She has also been trained and worked as a Vision Therapist for a period of time. Kylie currently provides community based therapy support services for clients with complex orthopaedic injuries, spinal cord injury and/or acquired brain injury in Barrie and surrounding areas.

Nicole Galeotalanza B.Sc., R.Kin, CCLCP

Canadian Certified Life Care Planner

Nicole is a Canadian Certified Life Care Planner, practicing case management with individuals who have suffered traumatic injuries. She graduated with a Bachelor of Science, Athletic Therapy from Concordia University in 2001 in Montreal, and then completed the Case Management Certificate program from McMaster University in 2009. She completed the Life Care Planning Pre-Certification Program through the University of Florida and was certified as a Canadian Certified Life Care Planner through the International Commission of Health Care Certification (ICHCC) in 2013.

Nicole is a member in good standing with the Ontario Kinesiology Association, College of Kinesiologists of Ontario and the International Association of Rehabilitation Professionals. Nicole maintains an active case management practice, managing rehabilitation services for individuals with various catastrophic impairments including, but not limited to, Traumatic Brain Injury, Spinal Cord Injury, Chronic Pain, Psychiatric Disorders and Amputation for both children and adults. She has been completing Life Care Plans with Complex Injury Rehab since 2016.

Antonella Finateri, OT Reg. (Ont.)

Occupational Therapist

Antonella Finateri is a member of the Ontario Society of Occupational Therapists and registered with the College of Occupational Therapists of Ontario. She has provided occupational therapy assessments and treatment for individuals with short or long-term disability, workplace injuries, motor vehicle accident injuries, and veterans for over twenty-five years. She uses s a creative, client-centered approach with a strong professional interest in both neuro and mental health rehabilitation.

Antonella is experienced with administering a variety of physical, cognitive, vocational, and psychosocial assessments. She is certified in Cognitive Behavioural Therapy, the Progressive Goal Attainment Program, in Microcurrent Point Stimulation and as a Certified by BrainFx Assessor. Antonella has been an authorizing clinician for Assistive Device Program funding for wheelchairs, walkers, and scooters through the Ministry of Health since 1990, and regularly attends workshops aimed at improving her professional skills and competencies.

Amanda Buschau, OT Reg. (Ont.)

Occupational Therapist

Amanda Buschau is a registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario. She is passionate about providing occupational therapy treatment to adults with concussions and other acquired brain injuries including stroke, mental health conditions, chronic pain and orthopedic injuries. She is experienced in conducting both discharge and in-home assessments that include assessment of attendant care needs, home safety, and comprehensive physical, cognitive and psychosocial assessments. Amanda is committed to collaborating with her clients to help them achieve their unique goals.

Amanda is a BrainFx 360 Administrator and has completed a variety of training including NDT Introductory Workshop in the Management of Adults with Stroke, Non-violent Crisis Prevention Intervention (CPI), Applied Suicide Intervention Skills Training (ASIST), Chedoke-McMaster Stroke Assessment, P.I.E.C.E.S. and Gentle Persuasive Approaches in Dementia Care.

In her spare time, Amanda enjoys singing, belly dancing, yoga and is trying out stand-up comedy to an audience bigger than her two cats.
